|Case/Care Management Services||Bridges to Health (B2H) is a Medicaid Waiver Program designed to supplement NYS Medicaid state plan services to populations of foster care youth, their families and caregivers. Foster care children and youth may have medical/physical, emotional and/or developmental challenges. The program works to support children and caregivers in the lowest level of care possible including return to home and/or adoption. The goal of the program is to support youth as they reach their highest level of potential, attain permanency and develop social and emotional competencies that provide for healthy, permanent families. |Criminal Justice Services||Provides a toll free 24 hour hotline to report information on missing children which is immediately shared with law enforcement agencies. |Crisis Intervention Services||This mobile crisis service assists individuals of all ages through mental health and substance abuse crises by providing them with alternative community services. They also provide telephone consultation and information to family/adults experiencing crisis and also professionals who are working with such individuals. Face-to-face crisis assessments is performed after screening of appropriateness. Crisis intervention and stabilization services can be given on-site at the individual’s location in the community. Care management follow-up is used specifically to facilitate transition from crisis intervention and stabilization to other agencies in the community for ongoing treatment and support. If individual require additional evaluation for inpatient treatment; referrals to services are available to them, such as outpatient mental health services, emergency department, housing, child services, etc.

|Education/Prevention||The Healthcare Consortium Kids in Motion program is a Childhood Obesity Prevention Program funded by NYS Department of Health. The goal is to get kids moving more, eating more fresh fruits and vegetables, and getting less screen time (TV and computers). This school and community-based program, has a front line presence in the lives of children in the K-5 age groups. The aim of The Healthcare Consortium Kids in Motion program is to reach out to kids who may feel a little left out of the sports team mentality. Not everyone has to be a competitive athlete, yet everyone needs to find something that moves them, quite literally, so they can have positive feelings about exercise and movement.

|Mental Health Services||The Child and Adolescent Mobile Team (CAMT) provides an immediate community-based crisis response service to any family with a child-adolescent in Albany, Rensselaer or Schenectady Counties, under the age of 18 (or older if still in day school programming) experiencing a serious emotional, psychiatric or behavioral crisis. The team will respond to homes schools, community agencies as requested by family members or law enforcement. A multidisciplinary team will provide consultation, information, crisis assessment and intervention, and facilitation of alternate levels of care directly to the families during crisis episodes, whien it is most likely to have a significant impact, and next day phone support to help the family gather knowledge and resources to prevent-deal with future crisis episodes.
