The Visiting Nurse Association of Albany Certified Home Health Agency (CHHA) provides short-term intermittent skilled nursing as well as physical, occupational and speech therapies, and home health aide services to qualified patients in their homes.
Albany, Columbia, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington
Referral required from patient's M.D, hospital discharge departments, community agencies, or family-caregiver. Patient's M. D. must sign orders for home care, VNA does assessment, insurance provider(s) approval needed. Family-caregivers are also involved. Initial point of contact is VNA patient acceptance-intake department. Call for information.
