Description of Service
Bridges to Health (B2H) is an OCFS Medicaid Waiver Program that provides supplementary services to children in foster care. These services are specifically tailored to support the health and well-being of children in foster care with a serious emotional disturbance, developmental disability, or medical fragility. Northeast provides Health Care Integration and home/ community based services.
Thirty-five county service area throughout the Capital Region and in New York State: (Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster, Westchester [in the Hudson Valley Region]; Clinton, Essex, Franklin [in the Adirondack Region]; Albany, Columbia, Fulton, Greene, Hamilton, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, Washington [in the Northeast Region]; Nassau and Suffolk)

Application procedure, DSS or DJJOY referral required.
Diagnosis by a qualified medical professional attesting to diagnostic criteria for a serious emotional disturbance, developmental disability, or medical fragility.
Youth may only be referred through a local department of social service, or the Department of Juvenile Justice and Opportunities for Youth.
Disabled - Developmentally
The youth must be in foster care, Medicaid eligible, 18 or younger, residing in a home, or agency, with less than 13 beds and meet the diagnostic criteria for serious emotional disturbance, developmental disability or medical fragility.

Mon-Fri 8:00 am-5:00 pm (Evening and weekend service hours are available, service hours are flexible to meet the needs of enrolled youth and caregivers)
