Description of Service
Provides those who are chronically ill or who have physical disabilities the freedom and flexibility to hire, train, and supervise their caregivers so they can remain at home, participate in the communities where they live, and avoid a nursing home or group home placement. The consumer recruits, interviews, hires, trains, and dismisses their own staff. Coordinators can provide some assistance and advice on how to maintain a good supervisor-worker relationship.

Must have medicaid, Call for details.
For CDPAP services call the Conflict Free Evaluation and Enrollment Center (CFEEC) at 1-888-401-6582 for the initial evaluation to determine eligibility for Community Based Long Term Care for more than 120 days and assistance in the enrollment process.
Anyone with Medicaid, Fedelis, CDPHP or private insurance that covers these types of services and who needs personal or home care assistance.
