Description of ServiceBridges to Health (B2H) is an OCFS Medicaid Waiver Program that provides supplementary services to children in foster care. These services are specifically tailored to support the health and well-being of children in foster care with a serious emotional disturbance, developmental disability, or medical fragility. Northeast provides Health Care Integration and home/ community based services.

Additional RequirementsDiagnosis by a qualified medical professional attesting to diagnostic criteria for a serious emotional disturbance, developmental disability, or medical fragility.
Helpful TipsYouth may only be referred through a local department of social service, or the Department of Juvenile Justice and Opportunities for Youth.

EligibilityThe youth must be in foster care, Medicaid eligible, 18 or younger, residing in a home, or agency, with less than 13 beds and meet the diagnostic criteria for serious emotional disturbance, developmental disability or medical fragility.

Specific Hours: Mon-Fri 8:00 am-5:00 pm (Evening and weekend service hours are available, service hours are flexible to meet the needs of enrolled youth and caregivers)
