General Information
Description of ServiceEducational Home Visiting provides home visits conducted by trained family support workers to enhance parent-child interactions and promote positive child growth and development. Families are enrolled in services early in pregnancy or immediately after birth to assist in promoting a healthy pregnancy, labor and birth. Curriculum-based home visits provide parents with skills needed to raise healthy resilient children. Families are provided with information referrals and connections to other community supports (i.e. WIC, health insurance, continuing education, literacy services.) Community Health Nurse visits continue for the first year.

Intake RequirementsSchenectady parents are enrolled prenatally or before the child reaches 3 months of age. Services continue unitl the child is 3 to 5 years of age. Initial home visit by Community Health Nurse assesses families' strengths and needs.
