General Information
Description of ServiceOffers an adult day health care program for individuals with a diagnosed health condition and a functional vision problem, to help maintain as much independence as possible. Services include nursing supervision, social work services, health education, occupational and physical therapy, vision rehabilitation, diabetes self-management education, nutritional counseling and therapeutic activities, including breakfast, lunch and transportation.
